Enrollment Forms for Counselor Training Courses. 

Form one and two need to be filled out and returned by the applicant with required $55.00 enrollment fee. 

Three recommendations are required. Please print out three Recommendation Forms and have forms completed and mailed by three individuals who are non-family members.
